Well, to my pleasant surprise I just discovered that beating the game on Serious mode unlocks a new a difficulty setting called Mental.
Has anyone tried Mental difficulty yet? Anyone know the exact differences between Serious and Mental?
I will try it out myself tomorrow... My hands are kinda weak though after playing all night ;-)
Thanks Croteam, finally a game that can live up to the legend of Doom 2 in my eyes (and I consider myself one of the biggest Doom fans out there - still play it reguarly).
And this extra secret makes it all the more sweet. Awesome game! I am in heaven!

OH MY GOD!!!
I beat it the first day i had it on the "Serious" setting and to be honest I thought it was sort of easy, only in terms of killing enemies. Except when there are 100000 around you ;). I so glad the game is so linear and doesnt have a lot of twists in the plot like most games. The only hard part I had was trying to figure out how to beat the last guy and that didnt take too long.
But anyway I'm writing to say that the "Mental" difficulty is INSANE! If anyone beats this I will bow to them I've beaten the first 2 maps but, damn, its getting REALLY hard. For anyone who wants to know why its so hard just play it on the setting and you'll see. The only thing I'll give away is that the enemies disappear and re-appear. Thats enough to really freak me out. Thank god this game is bright and not like Quake1 and dark or cause I would seriously shit myself about 4 times a minute if it was in a setting like Quake.

Yes, Decoy is correct. *In 'Mental' the enemies disappear and reappear. *It is mind-blowing and very difficult to play, but a blast to play regardless...

WOW, that is soo cool. I beat Normal yesterday, looks like I will have to give this a shot. What if you beat it co-op in Serious, does that still unlock the Mental setting?

I'm not sure, but there a lot of coop games on gamespy so go play and see.

dont be emarassed, its still hard as hell. Anyone who beats serious, whether its soop or sp should be proud.

I played the training mode before I started the regular game.
Beating the Training Mode on Serious will also give you Mental Mode.
